Home and Community-Based Services (HCBS) are a range of supportive services and programs designed to help individuals with disabilities, including intellectual or developmental disabilities, live more independently and participate in their communities. These services are provided in non-institutional settings, such as the individual’s own home, a family member’s home, or within the community itself, rather than in a facility like a nursing home or institution. HCBS programs are typically aimed at improving an individual’s quality of life, enhancing their ability to engage in daily activities, and promoting community integration.
Attendant Care is provided to help an individual with their personal hygiene, assist in dressing, and help with meals. Attendant care can be received in the home or out in the community.
Respite services will provide short-term relief for the primary caregiver. These services can be provided any time of the day or evening the caregiver requests. The certified respite provider will come and help the individual meet their needs in the home or community.
Habilitation Services are designed to support individuals in developing and enhancing essential life skills, fostering independence, and promoting overall personal growth. These services encompass a range of activities aimed at improving cognitive, social, and adaptive skills, tailored to the unique needs and goals of the individual. Habilitation services can be provided both in the home and within the community, offering a structured and supportive environment for individuals to acquire and strengthen skills that contribute to their overall well-being and quality of life.

Yes! Family Partners prefers to work with the parent to identify a trusted family member or close friend to get certified to become their child’s provider. Family Partners will provide the Fingerprint Clearance Card application and the required training. Identifying a close family member or friend tends to be more successful for the individual receiving services than having an unknown person or pool of service providers give the in-home care.
Currently, parents can provide services to their children of any age.
A parent can provide attendant care and habilitation services.
